There is no Second Chance for True Friendship

Friendship will provide us with thousands of fun moments in life which we may memorize in our old age. But side by side, with times this relation may have moments of crisis. A misunderstanding may crop in and this amazing relation may start weakening.

Always be the first one to save the friendship. God forbid, if due to some unfavorable reason distance ever arises within this relation, we must not let our friend go. Hold his or her hand and apologize for any mistake done. There is nothing more pathetic in anyone’s life than losing a true friend.

Friendship is a relationship that is like a newborn baby. Friendship is always pure and like a bundle of joy which only needs more and more nourishment to grow. Never ignore true friends or take them for granted. We may come across various people at different phases of our life. Many may pretend to be our friends but never go by outward glitter.

At last, I wish to say that a good friendship is very difficult to come across. Therefore, we should appreciate this divine relationship having a base on understanding and feelings. We just need friends to live happily. Lasting friendship is a blessing for everyone.

We don`t need to pretend to be someone else when we spend time with our friends. They give us total freedom to be who we are in reality. We should always be grateful to people who make us happy. A true friend is one of the most precious possessions in anyone’s life.

Why Your Friends Are More Important Than You Think

Researchers and philosophers have explored in great detail the emotional dramas of love and family. But they’ve spent much less time pondering the deep satisfaction of a good friend.

A similar thing happens in our own lives, writes science journalist Lydia Denworth. When something’s gotta give, it’s often our friendships, which take a backseat to our family and work obligations—or our latest fling.

But that’s a mistake, she argues in her new book, Friendship: The Evolution, Biology, and Extraordinary Power of Life’s Fundamental Bond . In fact, research suggests that friendships can help us find purpose and meaning, stay healthy, and live longer. The intimacy, support, equality, and emotional bonds we have in our friendships are unique.

Her book honors the relationships forged through slumber parties, shoulders cried upon, and kindnesses that don’t need to be repaid. “The science of friendship gives you permission to hang out with your friends and call it healthy,” she says. “You’re not being indulgent.” In a conversation with Greater Good , Denworth explains why we need our friends and how to keep those connections strong—even in a pandemic.

Kira Newman: How does friendship change for people across their lifespan?  


Lydia Denworth:  When you’re very young, of course, your primary social relationship is with your parents or caregivers. But when kids go to school, they start to have deeper friendships that involve, first, doing things together, and then a deeper, shared emotional element. Then in adolescence, it becomes even more abstract and relational.

All the way through high school and college, friendships can feel easy because you are thrown into an environment where you have lots of same-age peers and the pool of potential friends is big. Also, when you’re an adolescent, your brain is as attuned to social signals and connection as it will ever be. You are really hyper-interested in social activity.


Then in adulthood, as people start to have jobs and maybe get married or have a family, it can become harder to spend time with your friends. Toward the end of life, we tend to come back around to having a little bit more time once kids are grown and careers and jobs are less demanding.


There are these transition points in life when it’s easier or harder to spend time with friends, but what is important for people to know is that friendship is a lifelong endeavor and that it is something that people should be paying attention to at all points in life. I think that people sometimes think (especially in their 30s and 40s), “I just don’t have time for friends right now,” and that’s a mistake.

If you get to be 65 and then now you’re ready to start paying attention to friends, well, it’s a little bit like stopping smoking when you’re 65. If you go from 15 to 65 and you smoke the whole time, it’s still better to stop than not, but some damage will have been done. And if you don’t pay attention to friends all the way along, the same thing is true. 

KN: You observe in your book that we tend to neglect our friendships when we get busy, more so than other relationships. Can you say more about that?

LD:  The reason we do that is that we feel more beholden to our family that we’re related to, and that makes plenty of sense—we’re legally and biologically connected to our family members. So, I’m not saying that we should be spending a lot less time with family. But we also feel that spending time with friends, instead of working, is indulgent.

My message is that it is not necessarily indulgent because having good, strong friendships is as important for yourself as diet and exercise, and so it’s something you need to prioritize. If you are forever canceling on your friends or failing to make a point of seeing them or talking to them or interacting with them, then you are not being a good friend and you are not maintaining a strong relationship. You need your friends to be there down the road. But you have to do the work along the way, or they won’t be there. Friendship does take some time, but that’s kind of good news because (mostly) hanging out with your friends is fun.

The second half of the story, though, is that it’s quite normal for there to be change in our friendships over the course of a lifetime, and that’s OK. Friendship does need to be a relationship that’s longstanding, but you can cycle through several longstanding friendships in the course of your life. So, it isn’t that you can only stay friends with the people you knew when you were young, of course, because plenty of people do make friends in adulthood and those can become closer friends.

If a relationship is not healthy or even if it’s just not serving you well—if it’s not positive, if it’s really draining, or if it’s lopsided and one of you is always helping the other but not vice versa—that’s not so great. I think people need to realize that it is OK to walk away from friendships that aren’t good ones.  
 KN: That seems like the flipside of all the amazing benefits that we get when we have strong friendships: There’s a lot of potential for pain when we have difficult, conflict-ridden relationships. 


LD:  Just like a strong relationship is good for you, a negative relationship is bad for you. Even an ambivalent relationship is bad for you, it turns out, biologically. 

An ambivalent relationship is a relationship where you have positive feelings and negative feelings about the person or about your interactions with them. And that’s true of a lot of our relationships—almost half. 


Researchers had a scale of one to five: How positive does this relationship make you feel, and how negative does this relationship make you feel? Anybody who was two or above on both things counted as ambivalent, which is really broad. You could be five on the good and two on the bad. What was interesting was that any relationship that was categorized as ambivalent seemed to generate cardiovascular issues and other kinds of health problems. 


It’s not as surprising that a toxic relationship would be bad for your health. But I think that the problem with ambivalent relationships, which a lot of us have many of, is more surprising. I think most people suspect that the good outweighs the bad, and so far (it’s early days in that research) it doesn’t look that way. 

I think that all this is a reminder of the importance of working on relationships—all of them, but including your friendships. There’s real value in a positive friendship.

If it isn’t positive, then you can do a couple of things. One is you can try to make it better, work on it, have a hard conversation, perhaps. Two is you quit and you say, “I’m not going to have this person in my life,” but that can be very dramatic. And three would be that you shuffle that friend to the outer circles of your social life. Maybe it’s not someone you can easily stop seeing, but if you don’t rely on them emotionally anymore, then that’s better for you. 


KN: Are there some practices you would suggest or steps that you take in your own life to put more time and energy into friendship?  


LD:  It really does just begin as simply as paying attention and prioritizing. I try regularly to plan to get together with my close friends and the people I care about seeing a lot. We all have relatively busy lives, but I, first of all, make an effort to make the plan, and then I make an effort to get there—to show up. I think showing up is a really critical piece of friendship, in every sense of the phrase. 


It could just be that you don’t have time to get together with someone for dinner for weeks, so you have a phone call and you catch up that way. Taking time to catch up on somebody’s life and hear what’s going on with them is an important indicator of it’s worth my time to know what’s going on in your life .


In addition, I think it’s useful to remember that science has clarified the definition of a quality relationship. It has to have these minimum three things: It’s a stable, longstanding bond; it’s positive; and it’s cooperative—it’s helpful, reciprocal, I’m there for you, you’re there for me . 


When you’re interacting with your friends, you should be thinking about your side of it. Am I contributing to that? Have I been helpful lately? When was the last time I said something nice or told somebody why I appreciated them or did something nice for someone? Am I a reliable presence in that person’s life? You can think about the way you interact with your friends as needing to fall into those buckets, at a minimum. 

The same thing goes for the online, as well: being positive, being helpful, showing up from a distance, whether that’s just checking in by text or sending a funny joke or forwarding an article or calling—making time. People have been stressed and anxious lately, so we need to be there and provide an ear to listen, a shoulder to cry on, even virtually. 


KN: Right now, people in many places haven’t seen their friends for months. What do we miss out on when we can’t be around our friends in person?  


LD:  There’s a richness to being with your friends in person, and it hits all your senses. So, we’re not getting any of the tactile sense of being with our friends, and there’s a difference when you see them on a screen vs. when you see them in person, although we don’t entirely know in neuroscientific terms what those differences are yet. 


One of the things our brains do automatically when we’re having a conversation with someone in person is this natural sense of “call and response,” that I’m talking, and then you respond, and then you talk and I respond. We are reading each other’s cues in a way that makes it easier to do that.

When you’re online, sometimes not only is there a little bit of an artificialness to the interaction but there’s literally a lag that’s built in from the technology, and that is quite off-putting for our brains. Our brains recognize that as a different kind of interaction, and they don’t like it very much. I think that’s one reason why some people are being driven crazy by Zoom. And if you have a group on Zoom, it’s very hard figuring out who’s going to speak next. There’s a way that we handle that with nonverbal cues in person that is harder to pull off virtually. 


When you’re in person, you can have a much more natural conversation. There’s an ease and a warmth and a naturalness that we get when we’re with our friends, and I think we really are missing the ability to hug them and high five—that’s big stuff that matters a lot. So, it’s a loss. 

That said, people are reporting a lot of positive experiences, even remotely. We’re being forced to interact virtually, but we’re getting a lot of benefits out of it. It’s not the same, but it’s a whole lot better than nothing. Limited though it is, technology has been a lifesaver in this moment. I can’t imagine what this would have been like if we didn’t have it. 


KN: What do you most hope people will take away from the book?  

LD:  That they will make friendship a priority, that they will call a friend and work harder on thinking about the importance of being a good friend, that parents will think about talking to kids about the importance of friendship and modeling being a good friend and prioritizing it. Parents are full of messages about achievement, and not as many messages about what it means to be a good friend, but I think it’s one of the most important skills that a child can develop. Through all our lives, the importance of friendship has been hiding in plain sight.

It turns out that healthy relationships actually contribute to good physical health. Having a close circle of friends can decrease your risk of health problems like diabetes, heart attack, and stroke.

Having strong social ties can also decrease feelings of loneliness, which evidence shows can take a toll on your longevity. According to a 2010 review, people with strong relationships have half the risk of premature death from all causes.  

Social isolation and loneliness are linked to a variety of health issues such as high blood pressure, substance abuse, heart disease, and even cancer.

Friends Encourage Healthy Behaviors

One possible explanation for those health benefits is that friendships can help you make lifestyle changes that can have a direct impact on your well-being. For example, your friends can help you set and maintain goals to eat better and exercise more.   They can also watch out for you and give a heads-up when any unhealthy behaviors (like drinking too much ) get out of hand.

Additionally, people are more motivated and likely to stick to a weight loss or exercise program when they do it with a buddy. It's much easier to get out and stay active when you have a friend by your side.

That friend may also suggest activities that you would not have considered on your own—thus, pushing you outside your comfort zone to challenge your anxiety.

Friends Provide Emotional Support

If you find yourself going through a hard time, having a friend to help you through can make the transition easier.

Research also shows that happiness is contagious among friends. One study of high school students found that those who were depressed were twice as likely to recover if they had happy friends. Likewise, kids were half as likely to develop depression if their friends had a "healthy mood."

Friends Help Build Your Confidence

Everyone has self-doubts and insecurities every now and then. But having friends who support you plays a big role in building your self-esteem . When people around you provide validation, it impacts your ability to love and appreciate yourself.

Supportive friends can help you feel more confident by offering praise and reassurance when you're feeling unsure. They'll shine a light on just how amazing you are and how much you have to offer others.

Friends Help You Beat Stress

Everyone goes through stressful events. If you know you have people you can count on, you may be less likely to even perceive a tough time as stressful. 

Spending time with friends can also help reduce stress . According to Harvard Medical School, "social connections help relieve levels of stress, which can harm the heart's arteries, gut function, insulin regulation, and the immune system."

Friends can also help you cope with stressful situations. According to one small study, when children hang out with their friends during a stressful situation, they produce less cortisol, a hormone released when the body is under stress.

A lack of friends can leave you feeling lonely and without support, which makes you vulnerable to other problems such as depression and substance abuse.

While we might all be pretty good at building online communities, our in person ones are seriously lacking. When we invest the energy in our friendships, and then encourage our friends to be friends with each other it fosters a sense of community.

A healthy community can help us feel more at home in our towns and cities When we're out and about and we see people we know—at the grocery store, at a local cafe, etc—it lets us know that we are part of something bigger and that we belong.

Friends Push You To Be Your Best

Ever heard some version of the phrase, "you are the average of the people you spend your time with"? The people we're friends with have the power to influence our own values and character. When you prioritize friendships with people who are generous with their time, help others, or are ambitious or family-oriented, you are more likely to develop those values yourself.

Great friends have the power to mold you into the best version of yourself. They see you and love you for who you truly are. They encourage you and push you to do better and be the person you want to be—your "ideal self."

There are many different components of friendship. If someone is loyal to you, honest with you, shares many of your interests, and is there for you when you need them, you would likely consider them a friend.

Many of the benefits of friends could be considered evolutionary—having a group of friends can create feelings of safety and social inclusion. Caring for others, and having others that care for you in turn, can help foster a collective purpose and feelings of self-worth.

Very good morning to everyone. Today, I would like to deliver a speech on friendship which will take us on a walk through the streets of nostalgia and relive the precious essence of friendship that we all have experienced. 

Friend. That one kid who stole your candy in school or the one student who gained more marks than you in college, or the topper who taught you everything just before an exam in university. Every friend that crosses our path in life always has something to give us, be it good memories, lessons, or just something worthwhile. True friends are always there for us through thick and thin, through good or bad, and through weal and woe. True friends support us in our decisions and correct us for our well-being. Through this speech on friendship, I am delighted to extend my gratitude to all such friends I am blessed with. 

All types of friendships have something or the other that’s unique and special, a best friend, an acquaintance, or a group of friends. Whoever that may be, a friend should always be the one who fuels our passion to flying rather than clipping our wings. As we grow older, making new friends seems to be a tough job as we create a comfort zone of our own. So, the ones who remain friends since childhood and never give up on each other are the ones who are valuable. 

Friendship shouldn’t be accompanied by jealousy, ego, or insecurity. A good friendship is one where people can share all their emotions heart to heart without the fear of being judged. Although friends have numerous memories to giggle about, friendship is not only about smiles or jokes. Friends should always respect each other, understand each other and support each other throughout. No one is perfect in this world. So, insecurities and flaws just make a person complete and friends are the ones who never judge each other over each other’s flaws or insecurities, rather support each other when they get agitated due to the same. Searching for the good in people, accepting their flaws, and staying loyal help people end up being good friends. 

Friends are very special as we choose them to be with us and include them in our lives. So, one must make sure that they choose good ones. In this busy world, people tend to drift apart from each other due to work or other engagements. But a lot of us, especially in our college days in school days, find it difficult to make friends easily. Some of us may have lost friends, some of us have gained a lot of friends, some of us have a big circle of friends and some of us have a very small and tiny circle of friends, but at the end of the day, irrespective of how many friends and what type of friends you have, the only thing you will remember about your college and school days is the fun and folly that we have with our friends. Jealousy, ego and possessiveness are some of the human tendencies that each and every one of us has. Some of us might be judgemental, some of us might not be judgemental but at the end of the day we are all human beings and we are all born to have both good qualities and bad qualities within us.

While making friends, we look at only their good qualities but during the course of friendship, we get triggered by their bad qualities and hence end up breaking the friendship with them. A strong friendship is that relationship where we understand both the good and bad qualities of our friends and help them grow together. If you enjoy laughing and having a good time with them, it is only human to be with them during their bad times and share their sorrows and grievances.

I would like to conclude this friendship speech by saying that there is no person on this planet who will be perfect. Imperfections are what make human beings beautiful and we have to celebrate these perfections and imperfections with grace. Making and breaking friendship is a natural tendency and neither of it should be the reason for you to lose faith in friendship. Friendship is one of the most valuable and precious bonds and each and every one of us should live through that before it’s too late.

The only memories I have of my school and college days are the fun and enjoyment that I had with my friends. Don’t walk in front of me, I may not follow. Don’t walk behind me, I may not lead. Just walk beside me and be my friend.

Most of us have friends, or at least one friend—someone we spend time with, someone who knows us better than others do, someone we can count on when the need arises. It’s been said that a friend is a gift that we give ourselves. Yet, how much time do we really spend thinking about those people who matter more to us than all the other people we meet and interact with throughout our life, sometimes including family?

There’s no doubt that a friend adds to the fullness of life. Authenticity , honesty, and trust are qualities we expect to find in a friend. There’s an understanding that the binding together of people in friendship helps each of us define and realize a meaningful life.

A good friend shows up no matter what. A true friend supports and encourages us, tolerates our shortcomings, accepts us unconditionally, and cares for us no matter what.

An expert in old age has explained that having social interactions and good friendships could be as important as exercise and diet for longevity .

Professor Rose Anne Kenny, the chair of medical gerontology at Trinity College Dublin, Ireland, and the founding principal investigator for The Irish Longitudinal Study on Ageing, told the ZOE podcast that humans have evolved to need social interaction like we've evolved to need food and water. Denying ourselves social interaction is as "toxic" as not eating or drinking, she said.

Kenny's research suggests that social interaction is good for us

The TILDA study has collected information on the health, lifestyles, and financial situations of 8,504 people in Ireland over the age of 50 since 2006. Kenny said that the study found that social participation, friendship , and social relationships are just as important as the other factors that we usually associate with longevity , such as exercise, diet, and not smoking.

She said that loneliness "triggers chronic inflammation ," which could be the root cause of many of the "big diseases" that often impact people in older age, such as cancer, heart disease, strokes, and dementia.

Inflammation is a natural process that occurs when the body produces chemicals to help address injury or infection, and also when you experience stress But chronic, or long term, inflammation is associated with increased risks of health issues such as heart attacks. Research suggests that people who are more socially isolated have higher levels of inflammatory chemicals in their tissues, which, over time, could lead to poorer health.

Researchers aren't sure whether this inflammation comes as a direct result of loneliness, or if it is caused by the increased stress that comes with less social interaction.

Regardless, Kenny recommended that you should "put as much effort into building friendships as you do choosing your food or selecting the physical activity you like," to reduce stress and inflammation, and therefore potentially reduce the risks of poor health.

Professor James Coan, author of a study that found holding hands with someone you are close to relieves pain , previously told Insider: "Relationships are like stress relievers. Because they calm your body down, they take you out of fight or flight mode, and back to equilibrium. That's how we think it works."

Volunteering and group exercise are great ways to combat loneliness

Kenny said that an easy way to make friends or increase your social interaction is to make all your hobbies social, so doing a yoga class with others instead of alone at home, or joining a choir.

She also said that volunteering on a regular basis has been associated with a better quality of life, less physical illness, and less depression, no matter when you start. And you don't need to be happy to begin with, because volunteering "independently influences your health," according to Kenny.

One 2020 study found that volunteering is associated with a positive change in wellbeing, no matter whether participants were happy and healthy or not when they began to volunteer.

It's important to note, though, that the quality of social relationships matters, and you shouldn't prioritize those that don't make you happy.

"If a friendship or engagement with a family member is strained or unpleasant, that is not good for us," Kenny said. "We find triggers a stress process."

Healthy speakers can say 2–3 words per second and usually produce an error only about once every 1,000 words. To limit the number of errors we make, we are continuously monitoring our own speech. While speaking is easy, the brain process of monitoring our own speech is quite complex. In this article, we outline the process of selecting a word, understanding what happens when a speech error is made, and what could happen if the parts of the brain responsible for monitoring speech are damaged.

Speech and Speech Errors

We speak every day. We talk to our parents, our friends, and even our pets! Healthy, fluent speakers can say 2–3 words per second, which are selected from tens of thousands of words in the brain’s memory word bank—which holds over 50,000 words in adults! However, every now and then, our tongue slips and we make speech errors. Luckily, this only happens about once every 1,000 words we say [ 1 ]. The process of catching errors and correcting ourselves when we talk is more complicated than it seems. Take, for example, naming a picture of a cow. When we think of a cow, we generally think of a mammal that lives on a farm and makes milk. Even though the word “cow” comes to mind, it may not be the only word. We may accidentally say the word “horse.” The human brain has mechanisms in place to help us quickly realize when we are about to make a mistake. Researchers who study how we speak call this type of control speech monitoring .

Before we can understand how exactly we monitor our speech, let us break down what happens in the brain when we want to speak.

When we want to say a word like “cow,” we first need to think of the concepts relating to that word, such as “farm,” “milk,” and “mammal.” However, the concepts associated with “cow” can also be associated with other words. We may also think of a horse, cat, goat, or pig when thinking of the same concepts ( Figure 1 ). The word representations for all these other animals and the related concepts are all active in the brain when we want to say “cow,” which can make it difficult for us to select the correct word. Then, to say the word aloud, we need to access the sound representations (called phonemes ) that correspond to the words we want to say. All of this is happening very quickly in our brains, every time we speak!

  • First, we see the picture and think of all the concepts associated with it. In parallel, other words come to mind that are associated with those same concepts. To say the word aloud, we then select a word representation and its phonemes. Meanwhile, our brain is monitoring our speech before (inner speech monitoring) and after (outer speech monitoring) we speak. At any of these steps, an error can occur. A mechanism inside the MFC monitors errors and resolves conflict within each stage.

However, speech production is not perfect. We all make mistakes when speaking, especially when there are conflicting messages in our brain about which word or sound choices may be correct. Any of the steps we just described can go wrong, leading to all kinds of speech errors. For example, if we select the wrong word—one that is related in meaning to the one we wanted to say (like saying “horse” instead of “cow”)—we made a semantic error . If, instead, we string together the wrong phonemes and say something like “cav,” we made a phonological error .

Understanding how the brain monitors speech to prevent errors is important because speech monitoring is such an important part of speaking! Some people lose the ability to monitor their speech, and researchers want to understand how speech monitoring sometimes breaks down. By studying how people speak and the types of errors they make, researchers have found two pathways that allow us to catch and potentially correct our errors before and after we speak. These pathways are known as the inner and outer loops of speech monitoring [ 1 ]. The inner loop allows us to monitor our words before we speak, to select the correct word to say (yellow in Figure 1 ). The outer loop monitors our speech after we have said words out loud and are able to hear whether or not we made an error (orange in Figure 1 ).

How Do We Study Speech and Error Monitoring in the Brain?

Researchers have learned about brain regions that may support speech monitoring and speech production by using brain-imaging techniques like functional magnetic resonance imaging (fMRI). fMRI measures brain activity by detecting changes associated with blood flow. When you are using a part of your brain, blood flow to that area increases. When you are not using a brain region, blood flow to that region decreases. Using fMRI, researchers found that two main brain regions seem to be particularly important in supporting speech monitoring. One is the medial frontal cortex (MFC), specifically the dorsal anterior cingulate cortex (dACC) and the pre-supplementary motor area (pre-SMA; Figure 2A ). The second main region is the posterior superior temporal gyrus (pSTG; Figure 2B ). The engagement of the pSTG in speech monitoring was discovered using tasks in which people’s speech was distorted while they were speaking [ 2 ]. This research suggests that when your speech is distorted, like when you hear yourself speak underwater, your pSTG is more active compared to when your speech is not distorted, like when you hear yourself speak above water. In contrast, fMRI studies showed that the dACC and the pre-SMA are activated when we hear feedback from our normal non-distorted speech, and when there is a lot of conflicting information in the brain. In addition, other brain regions are known to be very important for speech production, including the left prefrontal cortex and the left posterior middle temporal gyrus ( Figure 2B ). As you can see, many brain regions are important in speech production!

However, even though fMRI is great at telling us which parts of the brain are active, it cannot tell us precisely when these regions are active. This is really important given how fast we speak! To answer this question, researchers use another technique called electroencephalography (EEG) to record electrical activity from nerve cells in the brain, by placing electrodes on the scalp. EEG shows changes in brain activity. Scientists found an electrical signal coming from the MFC that starts to rise before we start speaking. Because it starts before we can hear ourselves speak, this signal may be associated with the inner loop of speech monitoring. Because it was first observed when speech errors were about to be made, researchers call it the error-related negativity (ERN), but it is also present before correct speech, only smaller [ 3 ]. You can see the ERN in Figure 3 .

Why Do People Lose the Ability to Monitor What They Say?

Just like we need to eat our vegetables and exercise to be strong and healthy, our brain needs lots of oxygen and nutrients to function. Blood carries oxygen and nutrients to the brain through arteries. Unfortunately, sometimes these arteries get clogged or break and cause a type of brain-tissue damage called a stroke, due to lack of sufficient oxygen or nutrients to a brain region. If this region is an area of the brain that is important for language, some people may develop difficulty speaking or understanding what is said to them.

When people lose the ability to understand or express speech resulting from damage to the brain, it is called aphasia . Some people with aphasia may no longer be aware of the speech errors they are making. Those people might find it more difficult to recover their language skills—researchers have found that the ability to detect speech errors predicts how well someone with aphasia will benefit from speech therapy [ 4 ]. This tells us that, to improve their speech, people with aphasia first need to know they are making errors. When we are trying to learn anything new, the only way for us to improve is to practice and catch our errors. However, scientists do not yet fully understand why people with aphasia can lose the ability to monitor their speech. As explained earlier, certain brain regions are involved in speech production and speech monitoring. Could it be that certain brain regions are more important than others for making us aware of our errors?

In our study [ 5 ], we asked people with brain damage in the left and right prefrontal cortex and people with no brain damage to name pictures while we recorded their brain activity. We found that people with brain damage in the left prefrontal cortex made more errors and had slower verbal response times than people with no brain damage. However, these individuals had a larger ERN, detected in the MFC, when they made errors compared to when they responded correctly ( Figure 3 ). This told us that the left prefrontal cortex is probably not critical for the inner loop of speech monitoring, or at least not when we are simply naming pictures.

Where to Go From Here?

As you now know, a lot of work goes into producing and monitoring our speech. There is still a lot that researchers need to learn about how the brain monitors speech, and specifically about why people can lose the ability to monitor their speech after stroke. Current studies in our lab are investigating what happens when brain regions in the left temporal lobe of the brain are damaged in people with aphasia, the effects on speech-monitoring abilities and the ERN. If a person with aphasia no longer shows the expected ERN pattern, this may indicate that their ability to monitor speech is impaired. As you can see there is still so much we need to learn and discover. This is what makes language research exciting!

Speech Monitoring : ↑ The brain process of monitoring/controlling what we say—before and after we say it—to help us prevent or identify errors in our speech.

Phoneme : ↑ A sound representation stored in the brain. Phonemes (sounds) are strung together to make whole words like stringing together letters of the alphabet!

Semantic Error : ↑ A violation in the retrieval of a word in the brain by selecting a word that shares similar features to another (i.e., saying horse instead of cow ).

Phonological Error : ↑ A violation in the retrieval of a sound in the brain by selecting a sound that is similar to another (i.e., saying hat instead of cat ).

Functional Magnetic Resonance Imaging : ↑ A brain-imaging technique that measures changes in blood flow to the brain.

Electroence-Phalography : ↑ A brain-imaging technique that records electrical activity from neurons in the brain.

Error-Related Negativity (ERN) : ↑ An electrical signal in the brain that starts to rise before speech and is larger when speech errors are made compared to correct responses.

Aphasia : ↑ Language difficulty acquired from brain injury. There are currently eight different forms of aphasia depending on what part of the brain is affected.

Good morning and welcome to this Regional Meeting of the National Programme Managers for Lymphatic Filariasis Elimination and the Regional Technical Advisory Group (RTAG). 

Neglected Tropical Diseases (NTDs) significantly impact morbidity and mortality in marginalized communities worldwide, hindering development and impeding our efforts to leave no one behind.  

Our WHO South-East Asia Region bears the world's highest burden of many NTDs, including lymphatic filariasis (LF), leprosy, snakebite envenoming, soil-transmitted helminths and rabies. The number of individuals requiring interventions or care for NTDs in 2022 was 1.62 billion globally. 833 million, or 52%, were in our Region, with at least one NTD endemic in each of our 11 Member States.  

Since 2014, eliminating NTDs, including LF, has been a Flagship Priority for the Region, resulting in rapid and sustained progress. Notably, Maldives, Thailand, Sri Lanka, and Bangladesh have eliminated LF as a public health problem and are in the post-validation surveillance phase, to sustain the elimination status and further progress towards interruption of transmission of the disease. Timor-Leste is currently implementing post-Mass Drugs Administration (MDA) surveillance. India, Indonesia, Myanmar, and Nepal are actively implementing triple-drug therapy, known as IDA, to accelerate LF elimination efforts.  

Despite our progress and achievements, challenges continue to emerge in the LF elimination programme. While the endemicity map continues to shrink across the Region, many countries are struggling to get to hard-to-reach and vulnerable populations, and enhance and sustain community compliance to MDA. Innovative and evidence-based approaches are needed to identify the specific population groups that are not participating in MDA - such as adult males, urban settings and religious and cultural minorities - and address the socioeconomic and cultural factors to enhance their participation. 

WHO is publishing new M&E guidelines for LF, which demand operationally challenging LF surveys in the field to monitor and assess impacts of interventions. There is an urgent need of orientation on the new guidelines, capacity building and cross-sharing of country experiences. 

Due to suboptimal performance of the currently WHO-recommended Rapid Diagnostic Tests (RDTs) for both Wuchereria bancrofti and brugian filariasis, progress on implementation of LF surveys have been forced to delay in countries. Meanwhile, new RDTs for both species are being validated by WHO, hopefully to be recommended for operational use soon.  

Countries endemic for Brugian filariasis are struggling with persistent or newly emerging transmission, even after WHO validation of elimination as a public health problem. This is potentially due to its zoonotic transmission. Today we are pleased to have participation of Malaysia and Brunei Darussalam to share experience, cross learn and jointly strategize how to address transmission of Brugian filariasis across Asia.  

The absence of clear WHO guidance on effective post-validation surveillance strategy leaves countries independently navigating post-validation surveillance. A wealth of data has been generated, both from case surveillance and xenomonitoring, from several countries. This has however not always been thoroughly analyzed to deploy most optimum and cost-effective methods.     

Additionally, more and more countries are detecting LF infections among migrant workers from endemic countries in recent years. This demands cross-learning for the clear way forward across the Region and beyond. 

Finally, all nine countries are implementing morbidity management and disability prevention (MMDP) interventions. However, there is a critical need to further strengthen and sustain the quality of care as part of the PHC-centered health system, and even expand the scope of the MMDP services to encompass rehabilitation and psychosocial care. 

Ladies and Gentlemen, 

Over the next three days, you will review the progress of Member States, discuss challenges, and seek expert advice from RTAG members to address each of these critical challenges and accelerate the progress in all countries. I would like to ask RTAG members to carefully review the current situation and provide actionable guidance.
